We are continuing our work with the Royal United Hospital (RUH) to support the development and integration of Treatment Summaries (TS) across various cancer sites. This project aims to create bespoke TS templates that are both efficient for clinicians to complete and valuable for primary care teams in supporting patients living with and beyond cancer.

Our work:

Customised Templates: We are working with different cancer teams to design TS templates that are tailored to the needs of each specialty while maintaining a consistent approach.
Ease of Use: A key focus is ensuring that templates are quick and straightforward for clinicians to complete, reducing administrative burden while maintaining high-quality information.
Primary Care Engagement: We have sought input from GPs to ensure the summaries provide relevant and actionable information, making them as useful as possible for ongoing patient management.

Efforts will continue to refine templates based on team feedback, address system integration challenges, and provide support for embedding TS into routine practice. By improving the structure and usability of these summaries, we aim to enhance communication between hospital teams and primary care, ultimately benefiting patients as they transition out of secondary care.We are pleased to confirm that SWAG has approved funding for this role for a further 12 months, allowing us to complete this project and expand our focus to other cancer-related initiatives in primary care.For further updates or support related to TS implementation, please get in touch.
